Short Description The case manager provides crisis intervention, consultation, referrals, and brief interventions to patients. The role also completes follow-up with patients to assist in treatment linkages and navigating complex social service applications such as housing applications, disability enrollments, etc.Previous acute care experience preferred.
Education RequirementsBachelor Degree -Social Work/Registered Nurse required.
License/Certification RequirementsThe New Jersey Pay Transparency Act requires disclosure of the pay range for this position.
