Provide intensive case management to children and youth referred by the Department of Children and Families (DCF) to the Comprehensive Foster care program (CFC), with the goal of long-term stability and permanency. Case Manager is responsible for providing intensive case management supports to children and adolescent youth assigned to CFC foster care homes including day-to-day functioning of the home, advocacy, and crisis intervention. Participate in the screening of potential youth and preparing for their placement in the CFC program, including contact in advance with all collaterals, preparation of placement paperwork, and referrals to health/therapeutic providers in advance of placement to provide for continuity of services. Visit each assigned foster child a minimum of one time weekly.
